1. Download and complete the Substrate order form or Antibody order form (pdf  files -require Adobe Reader).

2. Fill the order form and Fax to: 706-542-4412 with the heading “ATTN: CarboSource Services” or send as an email attachment to: carbosource@ccrc.uga.edu

3. A contact name and email address is required.

4. You will receive an order confirmation by email within two working days. If you do not, please contact us immediately.

5. We will confirm your order by sending you an invoice number via email or fax (including the list of items and total price).

6. Orders will be sent within 5 business days after the total payment notification: send purchase order number (PO#) from your part and include CarboSoruce invoice number in the payment notification.

7. Payment options include check, money order, wire transfer and credit card.
